To help you maintain optimal eye health, the plan covers one routine eye exam every 24 months at 100% with no copay required.  Additionally, the plan provides a hardware allowance of up to $100 every 24 months to help cover the cost of prescription eyeglasses or contact lenses.  By visiting an in-network provider, you can maximize your benefits and significantly reduce your out-of-pocket expenses for high-quality eyewear.
