First Things First…Enroll and Choose Your PCP
Before your coverage can begin, you must complete an Aetna QPOS Enrollment Form and choose a Primary Care Physician (PCP) for yourself and each covered family member (enter the Primary Medical Office ID Number, not the provider number, on the form beside the information for each person to be covered). If you do not pick an Aetna PCP, benefits will be paid at the lowest level.

Finding an Aetna Doctor, Specialist, or other Provider is Easy!
You can call the Fund Office at 215-735-5720 (1-800-531-1199 outside of Philadelphia) or Aetna Member Services at 1-888-533-2195. You may also search on the web by visiting Aetna DocFind.

A Word about Your Aetna ID Cards

After your enrollment has been processed, you—and each covered dependent—will receive an Aetna ID card. The cards are mailed separately to each person. As soon as you get your ID card, check it to make sure it shows the Medical Office number for the PCP you chose. If the information on your ID card is not correct or if you do NOT receive Aetna ID cards, call the Fund Office at 215-735-5720 (or 1-800-531-1199 outside of Philadelphia) or Aetna Member Services at 1-888-502-3862.
