If you have 5 years of service, you may be eligible for limited health care and death benefits of the plan during retirement if:

You retire with a pension from the Pension Fund for Hospital & Health Care Employees, or 
You retire at age 65 or older with 15 or more years of Benefit Fund coverage.

The Benefit Fund office will describe these benefits at retirement if you are eligible.

Retiring Soon? 
If you’re thinking about retiring, contact the Benefit Fund office three months before your potential retirement date to ensure that your retirement benefits start on time. 
The Fund office can provide the appropriate forms and answer any questions. 

Medical/Dental/Vision – Your active member coverage as well as any Dependent coverage ends on the last day of the month in which you retire. If you or your Dependents do not qualify for health care Coverage as a retiree, you may be eligible to Continue coverage for a limited time through COBRA at your expense. 

Death/Accidental Death & Dismemberment (AD&D) Coverage – If you don’t qualify for a retiree death benefit, you may continue your death and accidental death and dismemberment (AD&D) coverage (50% of the coverage amount then in effect, up to a maximum of $5,000). You must pay for this coverage. The cost of this continued coverage is determined by the Trustees. 

Disability Insurance – Coverage ends on date of retirement. You cannot convert this coverage. 

Pension Plan – Depending on your age and years of service, you may be eligible to receive a benefit when you retire. Contact the Benefit Fund office for details about eligibility and benefits under this plan. 

Address/Personal Information – During your retirement, be sure to update your address and personal information, including beneficiary designation, to reflect any changes, if you are eligible for benefits. 

Before You Retire…. 
Prior to your retirement, you may want to contact the Fund office for an estimate of your pension benefits. 
Make sure the Fund office has your correct home address on file. 
Consider updating your Will to protect your family. 
If you move after retirement, please provide the Fund office with your new address.
