Here are some issues to consider: 

Medical/Dental/Vision – If your dependent(s) had coverage under your deceased spouse’s plan, you may enroll your eligible dependents under the Fund’s plan.

Death/Accidental Death & Dismemberment (AD&D) Coverage – You may change you Beneficiary for your life and AD&D Insurance. 

Disability Insurance – No effect on this benefit. 

Pension Plans – If your spouse was your named beneficiary, you may want to change your beneficiary designation. If your deceased dependent was your primary beneficiary, you may want to change your beneficiary designation. 

Address/Personal Information – Be sure to update your address and personal information, including beneficiary designation, to reflect any changes.

What You Should Do 
To change your benefit elections, you must return your forms to the Benefit Fund office within 30 days of the death of your spouse or dependent child.
