
You should inform the Benefit Fund of any Change in your address as soon as possible. If you change your residence, here are some issues to consider:
Medical/Dental - You may change your medical plan option only if you move out of your plan’s service area. You may not enroll, stop coverage, or change your level of coverage (e.g. from family to you only).
Life Insurance - Changes are permitted only if there is a change in your marital status or in the employment status or benefits eligibility of your spouse or your dependents.
How To Make Changes
Contact the Benefit Fund for the appropriate forms and answers to any questions. Keep in mind that the change you request must be consistent with the event and the change must be made within 30 days of your move.
Accidental Death & Dismemberment Coverage - Changes are permitted only if there is a change in your marital status.
Disability Insurance - No effect on this benefit.
Pension Plan - No effect on this benefit.

Contact your local post office for address change forms.
Please provide the Benefit Fund with your new address.
Be sure the Benefit Fund has your correct emergency contact information.
